Form I-9 Employment Eligibility Verification - Fall 2025

After School Matters teens participating in Internships (ages 16+) are required to complete the Form I-9 Employment Eligibility Verification process to verify their identity and employment authorization hired in the United States before the start of programs.

After accepting your Internship offer, you must schedule an in-person verification meeting where you will present specific identity-confirming documentation to complete the verification process. Please schedule your appointment below.

The following are examples of the documents that are acceptable for I-9 employment eligibility verification. The documents you present must be an original document and unexpired; we cannot accept copies or photos.

The following are examples of the documents that are acceptable:

List A

Confirms Identity and Employment Authorization

OR

List B (1 item) and List C (1 item)

List B confirms Identity and List C confirms Employment Authorization

You only need 1 document from List A.

  • U.S. Passport
  • U.S. Passport Card
  • Permanent Resident Card

We can accept a receipt for a replacement of a lost, stolen, or damaged List A document.

OR

You must bring 1 item from List B confirming identity.

LIST B item (MUST HAVE A PHOTO)

  • Drivers’ License
  • State ID card
  • School ID card with photograph🚨(current school year only) 🚨

We can accept a receipt for a replacement of a lost, stolen, or damaged List B document.

You must bring 1 item from List C confirming employment authorization.

LIST C item

  • Social Security Card
  • Birth Certificate
  • Employment Authorization document issued by DHS

We can accept a receipt for a replacement of a lost, stolen, or damaged List C document.

Appointment Scheduling

Schedule ONE appointment by clicking on the location you prefer to visit for your I-9 Verification in-person meeting. If you cannot attend your scheduled meeting, be sure to cancel your appointment. You can reschedule your appointment for another available date and time.

Please plan for about an hour to complete the appointment.

Obtaining a Social Security Card:

If you don’t have one, you can apply for one here; or search for a Chicago-area Social Security Administration office near you using this tool.

To apply for a Social Security Card:

  • Find your birth certificate + ONE Evidence of Identity (i.e., Driver’s License, State ID, etc.)
  • Print and fill out Social Security Application
  • Apply online, in-person, or by mail

Obtaining a State ID:

If you do not have a State ID, you can apply for one here.

Schedule an appointment at Department of Motor Vehicles.

You must bring documents such as your Social Security Card and Birth Certificate. Your documents must verify your:

  • Signature
  • Date Of Birth
  • SSN
  • Proof of residency
